Before we
begin.

I am either: (a) an adult participating in Nord Rhythmic Academy LLC programs and activities; or (b) the parent or legal guardian of a minor participating in such programs and activities. For purposes of this Agreement, the adult participant or minor participant is referred to as the “Participant.”

I understand the nature of the activities and represent that the Participant has the necessary experience and capabilities and is in appropriate health and physical condition to participate. I understand that the Participant should consult a qualified healthcare professional before participating if there are any medical conditions, injuries, physical limitations, or other concerns.

I understand that rhythmic gymnastics, stretching, flexibility training, conditioning, tumbling, dance, and the use of apparatus or equipment involve inherent and other risks. These risks may include, but are not limited to, falls, collisions, muscle strains, sprains, fractures, head or spinal injuries, permanent disability, paralysis, and death.

Being fully aware of these risks, I voluntarily choose to participate or voluntarily authorize the minor Participant to participate in Nord Rhythmic Academy LLC programs, online and in-person classes, private lessons, camps, events, and related activities. I knowingly and voluntarily accept and assume all risks associated with such participation, whether known or unknown, foreseeable or unforeseeable.

To the fullest extent permitted by applicable law, I hereby release, discharge, covenant not to sue, and forever hold harmless Nord Rhythmic Academy LLC, together with its owners, members, managers, officers, employees, coaches, contractors, volunteers, agents, representatives, facility owners, and affiliated parties (“Released Parties”), from claims, demands, liabilities, losses, damages, or causes of action arising from or related to the Participant’s involvement in the activities, including claims arising from the ordinary negligence of any Released Party, except where such release is prohibited by law.

I agree to indemnify and hold harmless the Released Parties from claims, liabilities, damages, litigation expenses, attorney fees, or costs arising from the Participant’s involvement in the activities or from any claim brought by me, the Participant, or anyone acting on the Participant’s behalf, to the fullest extent permitted by applicable law.

For online sessions, I understand that I am responsible for providing a safe and suitable training area, appropriate equipment, adequate supervision of a minor Participant, and a reliable internet connection. The Participant agrees to follow the coach’s instructions and immediately stop exercising if experiencing pain, dizziness, shortness of breath, or unusual discomfort.

I acknowledge that I have carefully read and understood this Agreement, have had the opportunity to ask questions, and understand that I am giving up substantial legal rights. I voluntarily agree to its terms on behalf of myself and, when applicable, the minor Participant.
